I UNDERSTAND THAT IT IS MY RESPONSIBILITY TO ADD ANY AND ALL (TNC) ENDORSEMENTS TO MY AUTO INSURANCE POLICY. I UNDERSTAND THAT ANY AND ALL LIABILITY IS MINE TO BEAR. Silver Lining App(SLA) DOES HAVE ADDITIONAL INSURANCE COVERAGE FOR ME WHEN OPERATING AS A SLA DRIVER CONCIERGE AS REQUIRED BY EACH STATE LAW. I OFFICIALLY HOLD HARMLESS SLA OF ANY LIABILITY ABOVE AND BEYOND SLA INSURANCE COVERAGE, CONNECTED TO MY OPERATING MY VEHICLE AS A DC FOR SLA OR ANY OTHER TIME THAT MY VEHICLE IS BEING USED. I AM SUBMITTING ALL NECESSARY ATTACHMENTS INCLUDING MY MEMO OF UNDERSTANDING AND ACKNOWLEDGMENT OF VIEWING THE TRAINING VIDEO.
If you believe you will journeying on roads within your state that require a TOLL to be paid, please note that it is your responsibility to pay the toll at the 'commercial' toll rate when operating as a Driver Concierge for SLA.
Please Acknowledge Terms and Conditions provided by SLA. This must be done prior to consideration to operate as a DC of SLA

We are required to maintain a current background check on each of our Driver Concierge's which has to include a Motor Vehicle Report.
You have two options to comply with this requirement:
Option #1
CLICK HERE to complete your Federal Background check that will be submitted to SLA upon completion.
You may also choose to receive a copy. There is a small fee to do this.
Option #2
Once your background check is either completed via the link or uploaded,
DON'T FORGET TO HIT "SUBMIT" BUTTON BELOW TO FINALIZE YOUR APPLICATION. Thank you!
*Please note that at any time SLA may request directly FROM YOU, the DC, an authorization for release of your most current Federal Background information to comply with state laws.
You will be receiving a photo badge for the interior of your car to be clearly visible and displayed at all times when operating as a DC of SLA. You will be receiving 2 exterior window decals to be visible and displayed on your front windshield right corner and rear window right corner at all times, when operating as a DC of SLA. You will also be receiving magnetic signage for the Driver Door to be displayed at all times when operating as a DC of SLA.
Should you leave SLA by our request or a request of your own, several state laws REQUIRE you to return to SLA, within 14 days, all items given to you during your time operating as a DC. Please return items to:
